This is a form covering the agreement between Andorra and Spain (E/AND11) or Andorra and Portugal (P/AND3) for holiday journeys made by the insured person and/or their beneficiaries

You must request this form on the ground floor of CASS for the exact time period you will be away and this cannot be for longer than 25 days or 60 days (if you are an insured pensioner) starting from the date of request.

Should you require medical attention, you must present this form at the health centre, which must belong to the Social Security system of the foreign country with agreements, that will use it depending on his legislation.

Should you need to pay a medical expense and you do not have any form you may contact CASS, who will possibly send the form by fax or pay the expenses as long as you present all the invoices and/or prescriptions with as many details as possible (name of the patient, signature of the doctor, stamp...). CASS will reimburse in accordance with their liability tariffs.
